I've read all the posts I could find and I'm not sure how anyone has resolved this issue.
I am able to upload small files <10MB fine. I checked my ini file and the max is 999M so theorhetically I could upload large files. But it is giving me the Upload Required error
Suggestions?

php upload and post are correct and im still having the problem even with a 2 kb file any file i try to upload i get the popup UPLOAD REQUIRED!
no errors in the error logs
UPDATE: still having the same problem after it randomly stopped and now its back
