Amazon extension routes

I installed Amazon pay. The login part works partially and creates the local OC3 account, but then redirects to the home page.

Querystring is:
?route=extension/payment/amazon_login_pay/address

I checked the file system and extension/payment/amazon_login_pay_address.twig exists in the themes folder

No OC version posted. However, your shipping settings are improperly configured. Make sure the shipping extension module you are using properly tracks in the shipping ID and the shipping quote.

I am on OC 3.02.

I have the following shipping modules enabled.

Per Item
Free Shipping
USPS
UPS

My mods are

Custom written Avlara tax table lookups (this simply queries a free Avalara tax table by zipcode and replaces the tax variable )

All cookies have the secure and http only flags set (this should be in the core) (Amazon Pay did nto work prior to this change)
Index.php to / rewrite (Amazon pay does not work with this code removed)

Installed

amazon_pay_and_login_with_amazon.ocmod.zip

Amazon pay did not work prior to these installations.
