Hi, how exactly do I integrate opencart with a form? Here is a screenshot of my form:

I noticed when I installed OpenCart into my website it created a separate URL for my cart page: http://health-news101.com/ocart2/

Is it possible to ONLY have it for my checkout form?

One cannot just copy such Form-CODE, and use it for OC. OC does not consist of a range html/php - Pages only, therefore, such a Form could only be 'server-handled' like any regular HTML/PHP Form, mostly/usually sending any content, entered by a User, as eMail, to some account. But not, to be used/handled as part of OC, related to the OC Framework+DB, without beeing technically/functionally rewritten in Full !

That's, what I meant in the first reply, you won't find anybody here, doing this for you, or assisting you, for some time to come, just for free. :'(
