pathReplaces.php will handle it if set up correctly but you do have to clear out your cache for it to be effective.

Don't take that changelog as gospel. You're also going to have difficulty pulling specific bug fixes out of it. Scope is not well separated in commits so you'll get feature work mixed in with bug fixes.

If you're using the v2.x version you install it through the extension installer. If you're using the 1.5x version it's usually caused by uploading to the wrong location.
